CASE 4. EXCRETORY PROBLEMS. Taylor Swift
Taylor has been suffering from pain in the kidneys for two years. She has come to our hospital for a rutinary revision but she says that she feels pain when she urinates. In this revision we have to discover what is the disease of Taylor.
You can earn 200 XP points with this case.
1. You have 1,5 hours to solve this case and explain Taylor what is happening to her.
2. You have to complete in your notebook the following aspects of Taylor´s medical record. Use the following template in your notebook.
ASPECTS TO INCLUDE:
1. (20 xp points) As the problem of Taylor is related to the Excretory system. Explain what is the excretory system, related organs and roles.

2. (30 xp points) We have done an urine test and blood test to Taylor. The results says that the Urinary system of Taylor is damaged and the rest organs of the excretory system work perfectly.

3. (40 xp points) KIDNEYS. It seems that something is wrong in Taylor´s left kidney.

How does the blood get to the kidney from the heart? Check the arteries and veins that are connected from the kidney to the heart.
How many litres of blood are passing trought the kidneys per day?

4. (60 xp points) NEPHRON. The units that filter the blood in the kidney are the nephrons. But how does it work?.....................15 minutes
4.1. Make a drawing of the basic structure of a nephron with the parts.
4.2. Watch the animation of the activity of the nephron. Answer the questions:
How many nephrons do we have in a kidney?
Where is filtered the blood in the nephron?
What two different tubules can we find in a nephron?
What is the bowman´s capsule? Which is its function?
How do we call the duct that collects the urine after all the process?
5. (20 xp points) URINE. We have an urine test.Taylor is producing aroung 10 litres of urine per day. Is that common or may be it is a mistake in the medical record?
5.1. Write the typical normal volume of urine in your notebook.
We have checked that there was a part of blood in her urine. Is that common?
5.2. Complete your report with a concept map of the 5 main components we can find in the urine.

6. DIAGNOSIS.(20 xp) At this point, we have certain evidences about the two possible illnesses of Taylor, but you have to diagnose which one. Taylor says that she does not drink too much water.
